When you submit a form through my Facebook or Instagram advertising, or through this website, I collect the information you provide, which may include your name, email address, phone number, state of residence, and general information about the insurance coverage you are interested in.
If you later choose to apply for coverage, I may collect additional information necessary to complete that application, including date of birth, health history, and financial information. That information is collected directly from you at that time, not through my advertising forms.
I do not collect Social Security numbers, bank account numbers, or payment card information through any advertising form.

My services are intended for individuals 18 and older. I do not knowingly collect information from anyone under 18.
